The name is Stabsfeldwebel Jan (Janos) Gallik (can also be with one "l" letter), born 21.7.1892 in Alsósajó (Nižná Slaná, Slovakia)... I would like to update /add the information to my copy of 'Heldenwerk 1914-1918'. Additonally, could you also ask him if he is willing to share the picture he has of his grandfather with me?
