The Sacrifice Medal was created to recognize a member of the Canadian Forces, a member of an allied force, or a Canadian civilian under the authority of the Canadian Forces who, as of October 7, 2001, died or was wounded under honourable circumstances as a direct result of hostile action.
This is a link to the web site of the Governor General of Canada for this particular award, as far as I know, no info or pictures there have any copyright unless clearly specified from outside agencies.
Cheers!

This is the bogus 10th anniversary medal with award document. Many civilian and/or veterans' and/or political associations/committees bestow medals with very official looking documents and have their own also quite official looking stamps. These are unofficial (non governmental) awards.
This is a perfect example of such a medal.
