In the 1964 AF Officer Active List there is no officer with the last name as MINAHAN

All ranks can get these
The DSO is given for leadership only (non-combat related), the DSO is open to all ranks
Gone are the MM, DCM, DFM, AFM, DSM, CGM
There are no more officer only or enlisted only awards

The only reference I have (WP QuattroPro document) groups these in the category of Advanced Camp Awards, a standards reference has not surfaced as far as I know

1) Military Proficiency
The Military Proficiency is given to the top five percent of Cadets in each camp cycle, based on the
Individuals Combined Military Proficiency scores as prescriben by the Commander, Cadet Command
2) Leadership award
I can find no reference or criteria for what the Leadership Award is awarded
Your confusion is warranted as the Physical Proficiency, and the Platinum Medal Athlete award ribbons
are the same. The PP is a Unit Award, while the PMA is a Individual Award
