The Austro-Hungarian Red Cross decorations with war wreath were given sometimes for VERY strange reasons! I know of one to a German officer--for running a Siberian prison camp after the Tsarist guards all ran away and just... left them there... and awards to motor transport officers are not unusual.

With that THird Reich civil service cross, chances are this fellow was not a medical officer during the war.
