In my attempt to help out I present this translation for review an critique by the forum members.
During period while on front in Patriotic War he completed 36 successful combat missions
in accompanying Sturmovik aircraft gathering intelligence information on enemy. While in support role he could be depended on to provide cover for his lead aircraft, and as a result,
his lead aircraft carried out five aerial attacks and brought down two enemy aircrafts,
without a single loss to the bombers he was escorting nor to his own aircraft.
16.2.45 While in accompanying PE-2 over target he noticed ME-109, Comrade Skurikhin radioed and warned the lead aircraft, as a result of the battle and reliable cover brought down one ME-109, and after checking the status of his lead aircraft, left the battlefield.
He is well versed in the ?Airacobra? aircraft and is competent in utilizing its performance.
He is also a technical pilot through good training , which is why he fulfills his combat assignments even in adverse conditions , and, has not had a single accident.
Respectful to his senior officers. Strong political and moral convictions.
For completing 36 successful combat missions providing dependable cover for his lead aircraft,
which allowed his lead aircraft to bring down two enemy aircrafts- deserving of state award
order ? Patriotic War 1st class?.

Recommended for Order of Patriotic War 1st class. Through the chain of command ended up with the Order of Red Banner.

Gentlemen:
There is a document uploaded as part of this current post of research for Butikov I am curious about. The document appears to be a partially filled out award card with a rectangular stamp with the words наград вручена. Does the tamp indicate that the award has been presented or will be?
How does this document fit in to the award process.

on the citation where it refers to wounds and contusion in patriotic war I believe it says 3 wounds and lists Stalingrad front1943, 1st Ukranian front 194(?) and 2nd Byelorussian front .

If my translation is correct, Comrade Butikov repaired 18 breaks in communication lines while under artillery fire at great risk to his life and
in doing so maintained communication between the Division Commander and the Battalion Commander.
