What I consider an exception is the star I'm going to illustrate in this topic: it was made as a present to a veteran of the Civil War, in the first years of 20th Century, in an outstanding quality. The piece is unmarked, nor signed by the jeweller who made it (but it should have been someone mastering his art). The case bears no marks either.
