Capture of Königsberg Medal Document Help

Hi all

 

Would anybody be able to assist me in translating/decoding any of the information on the attached medal document? I am interested to find out who this medal was awarded to, including rank and/or unit if possible.

She's a Lieutenant in the medical service btw, Мед служба. The unit is Sanitary Independant Lvov military hospital if I am correct.

The last part of the unit mentioned is the Львовский военный округ so in full it's the Sanitary Independant Lvov Military District.
