I have managed to locate an image of a St George with a laurel branch. Not exactly the same but would indicate that the practice of these additions did happen.

The twig is a laurel branch and it signifies victory or heroism. When I get into town tomorrow I will check in my reference books to see if it is listed for any of the Imperial medals.

The clasp at the back is interesting, it seems a very bulky type of fastening compared to others I have seen. Has anyone come across this type of clasp before?
