Thai book about orders and decorations

Gentlemen, I looking for somebody with sufficient command of Thai language to help me catalogize the book in my library. I need translation of the title and, if possible, transliteration of the same, name of author(s), place and year of the publication and other relevant information contained in the impressum. I am enclosing the photographs of the cover and of the impressum.

This book is published by the medals department of the Secretariat of the Cabinet.  They republish similar, updated, books from time to time.  The second page shown is acknowledgements, or names of the committee members. The  publishing information is on another page (I don't have the book handy, but I have written it down before as below).

 

This is how I catalogued it :
