The obverse "For faithfulness 1939-1945" is rather inappropriate, since from what I can tell, this is for the May 1945 Prague uprising--continuing after the war had "ended." The "1st National Uprising" was crushed in Slovakia in 1944 when the approaching Red Army was bogged down in the Carpathian mountains for 9 months.

This bronze medal was established by the government resolution dated may 14th, 1946. It is quite common, non expensive, no fakes exist .
