Can someone help transliterate....or is this even German?

German it is not. A guess would be Italian?

I was told it might be Polish, but it looked similar to old German script. Maybe Lithuanian(?)

Bernhard's right. It is Italian.

The first line starts "Guardaci e ...", which is "look at us and ...", but I can't make out the rest.

I can't make out the first word on the second line, but after the comma is "con affetto", or "with affection".
