Soviet Translation help on memorial to family member

The pic is of a plaque on a memorial to partisan and my great aunt in the Ukraine. The was able to scan from a picture the plaque. I know it is hard to see but can anyone provide any information from it? Thanks for the help.

I don't recognize any of the writting. There is a small plaque placed on the base of the memorial that has the family name and a date of 1988.

Although of little help, the top two words are "Here is buried..." You really must supply a better picture.

Thanks. Your translation of the first few word helps tremendously. This is not a memorial it is a grave stone.
