Here are two examples to the same soldier. He initially received the Wound Badge in Black in late November 1941 for schrapnel injuries to his face a week earlier.
He spent the rest of the war recovering and spending time in the Regiment's replacement units until in December 1944, the Wound Badge in Black was upgraded to Silver for the same wound received 3 years earlier.

There are many variants of citations for Sports Awards from the more formal German Horseman's & SA Sports Badge to the less formal unit-level sports day certificates.
Here is an example of the latter for a member of 52 ID and signed by Lothar Rendulic and also the soldiers booklet/citation for the Deutsches-Reichs-Sportsabzeichen in Bronze...
