This medal belongs to the Princely Houseorder of Hohenzollern, not to the Royal (Prussian) Houseorder of Hohenzollern. The change of the reverse date on these medals seems to be unclear. The right date of the creation of the princely order was "5. December 1841". When some changes occured in the classes of the Princely Houseorder in 1852 (at the same time the Royal Houseorder was founded), the medals (silver and gold) were established. These had the right date "5. December 1841". From a time, not be known in detail, a new embossing tool was made with the wrong date "1. Januar 1842". How this could happend, is told in detail in Link/Gauggel "Fürstlich Hohenzollernsche Orden und Ehrenzeichen". It is to difficult to me to translate, so I can only show it in German here. Many thanks for that help! In between I got the translation. I was not aware, that this old osmanian writing is so difficult to read. It means His Highness Prince Heinrich. It is the medal for the Grandadmiral and brother of the Kaiser. Regards, Komtur.

As Sascha said before, any kind of medical officer (Sanitätsoffizier), who did service outside of Germany is for this ribbon bar impossible, because of the EK on white ribbon. Most of the nonkombattant EK we find on ribbon bars/order bars belongs to "officials" (Beamte). There were military officials and civil officials. Because of the same reason, as described for the medical officers, most of the military officials got the EK on the black ribbon. In the same way the Red Cross Medal and the Verdienstkreuz für Kriegshilfsdienst were given for any kind of "home service" and not only for special medical service. Therefore the combination of 1914 EK2 on white ribbon with these both decoration often belongs to civil officials, who did service in Germany for instance in ministries or administration. These rules are only for EK2 1914, in 1870 it was different. Regards, Komtur.
