Hello,
well I do not know for sure, but I have an (educated) guess: I think it is Nicaragua's Miguel Larreynaga Order (whatever grade, I would think at least Grand Officer for a full Admiral, http://www.medals.org.uk/nicaragua/nicaragua007.htm). It would make sense, since the GDR and Nicaragua had very close relations after the Nicaraguan revolution of 1979. The GDR sent lots of material, aid and money, I bet Ehm went there by ship at least once.
Two ribbons to the left of it, I am pretty sure, is Cuba's Ernesto Che Guevara Order First Class (red with one white stripe), so the Americas are near to each other. The yellow one with light blue stripe I haven't been able to figure out yet, but if the others are Cuba and Nicaragua, it should be something geographically close.
There were two GDR decorations with red ribbon, too, but these had devices on them, and they are already on his bar: 1) fourth row from top, no. 2, and 2) second to last row, no. 1.
